• Employee: increments $10,000 to

      $500,00 or 5x’s EE’s Annual Base Salary

• Spouse: (under age 70) 50% of EE election

      Increments of $5,000 to $250,000

• Child: Birth to 6 months: $1,000;

      6 months to 19 or 25 if (FT student:

      $5,000 or $10,000

• Guaranteed Issue Amounts:

     Employee: $100,000

     Spouse: $25,000

     Child: all amounts approved

Voluntary Short-Term Disability

• Short Term Disability:

     15th day sickness

     15th day injury

     Benefit Duration: 26 weeks

     60% of salary to maximum

     Monthly benefit of $6,000

Voluntary Long-Term Disability

• Long Term Disability:

     180 Day Elimination

     Monthly Benefit: 60% of salary

     to maximum of $10,000
