TECHNOSYSTEMS GYM REIMBURSEMENT POLICY

 

Objective:

Technosystems Service Corporation (“the Company”) encourages employees to achieve and maintain a healthy lifestyle through physical fitness

Eligibility:

Any employee, whether full-time or part-time, is eligible to receive the gym reimbursement.  Employees who have not yet signed up for a gym membership are also eligible to receive a reimbursement if the gym has an enrollment fee. 

Policy:

Employees will be reimbursed up to $15.00 per month towards the gym membership fee.  Employees are also eligible to receive up to $50.00 towards the initial enrollment fee if the employee enrolls in a gym facility after starting employment with the Company.  Employees will be reimbursed on a monthly basis, regardless of the type of membership (annual or monthly).

Procedure:

Employees are eligible to receive the monthly gym reimbursement through payroll.  The monthly payment will be made with the pay date on the 25th of the month.   

In order to get started, you must submit a receipt of the membership fee and/or initial enrollment fee, which includes the name of the gym, and phone number to Human Resources.  You will be then required to sign an an affidavit provided through Paycom by Human resources before the first payment is made to you.

Human Resources will audit receipts from time to time by asking participating employees to provide evidence that the membership is still current.  If an employee does not provide the requested documentation within five (5) days, the membership reimbursement will be stopped immediately. 

Employees are responsible for notifying Human Resources immediately in any change in his/her gym membership status. Failure to report the change in status while receiving the the Company reimbursement may be disciplined for falsification of company records.
